Advance Care Directives & Nurses: Education on Patient Discussions

Description: Who will make your health care decisions if you cannot? What does a "good death" mean to you?

 

These are questions to which patients should have an answer before entering a hospital. All nurses should be comfortable with discussing Advance Care Planning (ACP) and walking patients through completing an Advance Care Directive (ACD). No one can predict the future, making it important for every adult patient to have an ACD on file.

 

Join us to learn why there is need for your patients and for you to have ACP discussions with family. Learn how to fill out an ACD form! The more you learn, the better nurse you can be for your patients.
