- I am trying to add a course in ISIS and it keeps telling me that instructor permission is required. What does that mean? It means that you will need to have the instructor sign a course action form so that you can be manually added into the course. Complete the form, have it signed, and turn it in to the school's registrar for processing. Course Action Form
- Where can I get an official copy of my transcript? Transcript Request
- The University of Virginia no longer sends printed grade reports to your home address. You may access your grades through ISIS. If you have a need for a printed grade report, you may request that through the Main Registrar's Office in Carruthers Hall. Request Grade Report

Transfer Courses
The Transfer of Credit website will help you determine what courses will transfer to the University of Virginia from other institutions
Students MAY NOT transfer in courses that are a required part of their curriculum in the nursing school. Students may only transfer in courses that fulfill their general education requirements, with the exception of a second writing requirement. Students are REQUIRED to fulfill the second writing requirement at the University. If a course is not on the transfer of credit analyzers students must fill out a Transfer of Credit Form
On-Line Courses
I am trying to register for an on-line course in ISIS and it's telling me that the course is restricted to instructor permission. What do I do?
Online courses follow a slightly different enrollment process than live courses. Most online classes, as well as GNUR 550, 551 and 580, are restricted, which means that new students need to contact The Office of the Registrar with the 5-digit schedule number (course mnemonic and number are not sufficient). Returning students will be automatically registered for these (but not all) restricted classes based on NEUM, our internal database, which tracks students’ program of study. The advisor will enter the program of study into NEUM when meeting or talking with their advisees during the advising period. In order for everybody’s schedule to be accurate at all times it is extremely important for students to inform their advisor of any changes in their program of study, so that he/she may update it right away. January and Summer Registration must be completed by the student at the appropriate office. (See Schedule and Calendar)
Required Immunizations
Students in the School of Nursing are required to maintain immunization records. This means that students need to fax immunization documentation to Student Health (fax 982-4262). Immunization forms are available here. This form should state that your records should be forwarded to the School of Nursing. The form should be turned into Student Health.
Your REQUIRED immunizations are:
- If born before 1957, students need documentation of one MMR, a positive MMR titer, a history of MD diagnosis of mumps or sign a declination.
- HPV vaccine is be recommended for all females 26 years of age or younger.
- Yearly PPD test - All students are required to have had PPD screening by September 1. The SON will provide free PPD testing at McLeod Hall prior to the beginning of fall classes. New students are required to get a two-step PPD testing prior to enrollment.
- Tetanus - This vaccination must be repeated every 10 years. A Tdap or booster will be required if it has been two years or longer since the last tetanus booster.
- Two doses of Measles, Mumps and Rubella vaccination. If born before 1957, students need documentation of one MMR, a positive MMR tites, a history of ND diagnosis of mumps, or a signed declination.
- Polio vaccination
- Meningitis vaccination for undergraduate students or waiver may be signed.
- Varicella - students who have had the disease may report this on the form. If you have not had the disease, you will need to prove immunity.
- Rubella- If you are born after 1957, you will need a positive rubella titer to prove immunity.
- Hepatitis B- You must provide the dates of a series of three vaccinations (Twinirix or Hepatitis B) OR the date of a positive Hepatitis B surface antibody titer.
Students will be able to track their compliance with SON requirements on the Net Learning system. Students are encouraged to check their transcript to be sure that all requirements have been met.
Students who need to provide immunization information to employers or synthesis practicum sites should request this information from Student Health approximately 2 weeks prior to their report date. To have records sent, download and complete the release of information form and request the information to be sent to your employer or practicum site when completing the form. Requests should be faxed to Student Health at 434-982-4262.
Required Competencies
All school of nursing students are required to complete competency training. See next section.
Net Learning Web Based Competency Training
The School of Nursing and the University of Virginia Health System both utilize the Net Learning training system. Students are identified by their social security number. For most students, you will be automatically assigned modules to complete. If, however, you are a hospital employee, you may be required to self-assign modules since the School of Nursing administrator does not have access to your Net Learning Profile. Hospital employees will be requested to provide copies of their Net Learning transcripts to the School of Nursing Assistant Registrar every year for tracking purposes.
Returning Students Must Complete
Medical Center Compliance, Privacy and Security Retraining 2007 (all returning students - required annually)
Bloodborne Pathogen/Infection Control Retraining 2007 (all returning undergraduate, master’s and post master’s students) Required Annually
Quality and Safety Retraining 2007 (all returning undergraduate, master’s and post master’s students - required annually)
CPR (UPDATE REQUIRED EVERY YEAR.) UVa accepts only the American Heart Association AED certification for Healthcare Providers, you must have Adult, Infant and Child training. The School of Nursing offers free classes every fall prior to the beginning of classes.
Criminal Background Check (CBI)
At this point criminal background checks are only necessary when it is required by a clinical agency. Different clinical agencies may require students to undergo a criminal background investigation using their own vendor. However, in general the selected vendor for criminal background checks is Certified Background at http://www.certifiedbackground.com
Procedure:
- Please complete the vendor information and make direct payment to the vendor. The vendor will provide you with a passcode and a pdf file of the results.
- It is your responsibility to provide your CBI results to the agency requesting it within 24 hours after the first day of classes (which is also the day of final notification of clinical placement sites). To initiate completion of your CBI please contact the selected vendor, provide the required data, sign the release and provide payment to the vendor.
- Please be sure to keep a hard copy of your own pdf file for your personal records.
Instructions:
- Go to http://www.certifiedbackground.com.
- Click on "Applicants" (NOT "Students")
- In the Package Code Box, enter NI16 (letter N, letter I, number 1, number 6)
- Then you will see a page with information about our package and its contents, costs, etc.
Request to Increase Course Load
Students who wish to take over the maximum allowable hours during a semester (17 hours for undergraduate students, and 15 hours for graduate students), must get special approval from the Associate Dean for Academic Programs, Sarah Farrell. Students will complete an increased course load form as well as a course action form (a faculty signature to add the course will not be needed unless the course requires instructor permission) with the course information and schedule number included. The completed course action form should be stapled to the Request form and submitted to the School of Nursing Registrar for processing.
Graduation
All students in the School of Nursing are Required to turn in an Intent to Graduate Form to the registrar. Forms are available here.
Undergraduate students are REQUIRED to attach a copy of their VISTAA form to their Intent Form. You can access the VISTAA forms ISIS on the web. Enter VISTAA, "request a report", then "view the report", then print.
Graduate students do not need to attach documentation. Doctoral students are required to complete BOTH the blue form for GSAS and School of Nursing Intent to Graduate Form.

Activate Your UVa Computing ID and Email -
Go to http://virginia.edu/orientation/myuva/ and then click on the “Activate my U.Va Computing ID” link. Read the page and click on the “Take me to my accounts!” link at the bottom of the page and follow the directions. From then on you may access your email account on the MyUVA portal using the “Webmail” link on the right of the page. You need to do this BEFORE you will be able to register for classes through ISIS.
Course Offering Directory (COD) -
Go to virginia.edu/registrar and then click on the COD link on the right of the page. You will see all courses listed by schools. All classes and sections at the University of Virginia have a unique 5-digit numerical code (called a schedule number). You will need this schedule number to add or drop a class using ISIS. Please review your registration packet for the suggested plan of study for your program area. You will receive a letter detailing information about your faculty advisor. It is your responsibility to contact your advisor and set up a meeting with him/her to complete your plan of study. During this meeting your advisor will add your program of study in the database called NEUM.
Registration (via ISIS) -
Go to virginia.edu/registrar Then enter your Student ID (social security number) and click on “First Timer User”. Follow the directions, which include establishing a temporary password. Once this has been completed, a permanent password will be emailed to you. To retrieve the email, please click on the “Webmail” link on the MyUVA portal. Once you know your permanent password you will be able to log on to ISIS using the Student ID (SS#) and your permanent password. The direct link to ISIS is www.virginia.edu/isis. IMPORTANT: You will not see the log-in page if you have a pop-blocker. Please inactivate before using ISIS. ( To inactivate go to “tools” and then click on “allow pop-ups”). Please only use Internet Explorer or Mozilla Firefox with ISIS, do not use Netscape.
Advising Access Code -
Once matriculated into the School of Nursing, all students, including online students, will be required to meet/email/talk with your advisor on a semester basis. During this time your advisor will inform you of your unique advising code. You will not be able to register without this number.
Advanced Practice Recognition
If you apply for advanced practice recognition in another state, you may be required to provide course syllabi, assignments, and records of clinical training. Please make sure that you maintain these records, as the School of Nursing does not provide such documentation.
